Revival Home Health Care is committed to maintaining the privacy of your Protected Health Information (PHI). We are required by law to inform you of our legal duties and privacy practices concerning PHI.

Your Health Information Rights

You have the following rights with respect to PHI:

  • Obtain a paper copy of this notice: Even if you have agreed to receive the notice electronically, you are still entitled to a paper copy.
  • Request restrictions on uses and disclosures of PHI: You may send a written request to Revival. We are not required to agree to restrictions if they are not legally permissible.
  • Inspect and obtain a copy of your PHI: You can inspect and request a copy of your PHI contained in a designated record set while Revival maintains it. You may request the records through your physician or billing records. Revival may charge for the costs of copying and mailing.
  • Request an amendment to PHI: If you feel that your PHI is incomplete or incorrect, you may request an amendment. You must provide the reason for your request. Revival may deny your request, and you have the right to file a statement of disagreement.
  • Request an accounting of disclosures: You can request an accounting of disclosures of your PHI made by Revival after April 14, 2003, with certain exceptions. The first request within a 12-month period is free. Subsequent requests may incur a cost.

HIPAA Compliance

The Health Insurance Portability & Accountability Act (HIPAA) governs the confidentiality and security of health information. HIPAA ensures the privacy of your health information and helps improve the efficiency of the healthcare system. Revival Home Health Care, as a covered entity under HIPAA, adheres to regulations concerning the collection, storage, and sharing of your PHI.

  • Confidentiality and Security: Revival ensures the confidentiality and security of your health information.
  • Requests for Health Information: We comply with HIPAA standards when requesting or obtaining health information.
  • Sharing Information: Revival follows HIPAA privacy regulations when sharing information with third parties.

PHI includes any information related to your health status that can identify you, whether created or received by Revival or a third party (e.g., your physician).

Disclosure of PHI

Revival Home Health Care may disclose your PHI in the following ways:

  • Treatment: We may share PHI with other healthcare providers involved in your care.
  • Payment: We may use PHI to bill your insurance provider.

We are prohibited from disclosing your PHI unless required by law, or in the following situations:

  • Coroners and funeral directors in accordance with the law.
  • Judicial proceedings or if you are a member of the armed services.
