Preventing rehospitalization
30-Day Intensive
For rapid recovery, our expert case managers customize a month-long homecare plan. This comprehensive program helps patients overcome medical challenges of varying complexities, and includes a full suite of skilled nursing services, therapies, referral services, deliveries, daily monitoring and more.
Risk stratification
We review our population’s rehospitalization likelihood every day to triage those with elevated risks. Using our state of the art telehealth program, a team of collects and assesses the need for emergent intervention. Revival’s timely response prevent medical crisis and prioritize safe transition of care.
Medication matchup
As part of our assessment visit, we match the drugs present in the patient’s home to their prescriptions. This enables us to find errors, verify things with the doctor and get the patient on the right drug delivery regimen.
