Home health aides, once overlooked in economic headlines, have quietly become the backbone of New York’s post-pandemic employment recovery.

As of early 2024, New York’s employment numbers finally surpassed pre-pandemic levels. But that growth wasn’t fueled by tech, finance, or tourism. Instead, it was driven largely by a surge in health and social assistance jobs, specifically, roles in home health care and services for elderly and disabled individuals. 

These two sectors alone accounted for an astonishing 80% of job gains in the category, and without them, the city’s economy would still be lagging behind 2020 levels.

What makes New York’s home health care workforce so distinctive isn’t just its size — it’s the speed of its expansion. Since 2016, employment in these roles has doubled. Since 2009, it has tripled. Today, home health and personal care aides represent the largest occupational group in the state. Nationally, they are also near the top, rivaling retail and fast food workers in sheer numbers.

But here’s the twist: New York isn’t significantly older than the rest of the country. Its population of adults 65 and older is only slightly above the national average. 

So what explains the disproportionate rise of home care work in the state?

The answer lies in New York’s uniquely expansive Medicaid policies and its cultural shift toward aging-in-place. Through flexible eligibility standards and generous home-based care programs, the state has transformed what was once a safety-net service into a middle-class expectation. 

For families across New York, home health care offers convenience but also comfort, continuity, and control. Aging in place allows individuals to remain close to loved ones and maintain a sense of independence, which can be lost in institutional settings. And when care needs become more complex, skilled nursing delivered at home helps bridge the gap—offering clinical-level care in the same trusted environment.

A growing number of families now take part in programs like the Consumer Directed Personal Assistance Program (CDPAP), which lets Medicaid recipients hire their own caregivers. 

In 2023 alone, this program accounted for more than 240 million hours of care, more than double what it provided just five years ago.

This model works. Not only is it more personal and humane, but it’s also more cost-effective than nursing care. And with the state’s support, it’s now accessible to a much broader range of residents, from lower-income households to middle-class families planning ahead for elder care.

Home health care aides don’t simply provide medical assistance; they offer companionship, dignity, and daily support for those who need it most. Their work is physically demanding and emotionally complex, but it’s also deeply meaningful.

For many aides, especially those hired through consumer-directed programs, caregiving is more than a job. It might be their mother, uncle, or neighbor they’re assisting. This personal connection adds a layer of compassion that’s hard to replicate in institutional environments.

And yet, despite their essential role, these workers often operate in the shadows, underpaid, under-protected, and underappreciated. 

As home care continues to anchor New York’s economic recovery, the spotlight is finally starting to turn their way.
